This is why the rescue plan is not an optional extra. It is a legal requirement, and it must be in place before anyone enters a confined space.

A permit-to-work is a formal written system that authorises specific workers to carry out specific work in a specific confined space during a specific time period. The permit must be reviewed and re-issued if the conditions in the confined space change, if the work is interrupted for a significant period, or if the work is not completed within the time period specified in the permit.
Atmospheric Testing
Atmospheric testing is a critical step in confined space entry that is often skipped or carried out inadequately. Before any person enters a confined space, the atmosphere must be tested for:
Oxygen levels: Normal atmospheric oxygen is approximately 20.9%. Levels below 19.5% indicate oxygen deficiency, which can cause loss of consciousness without warning. Levels above 23.5% indicate oxygen enrichment, which significantly increases the risk of fire and explosion.
Flammable gases: The lower explosive limit (LEL) of flammable gases must be measured. Work should not proceed if the LEL exceeds 10% of the lower explosive limit.

The General Application Regulations 2007 require that rescue arrangements are established before any person enters a confined space. These arrangements must be sufficient to rescue a person who becomes incapacitated in the confined space without putting the rescuers at risk.
A confined space rescue plan must address:
Non-entry rescue: Where possible, rescue should be carried out without anyone entering the confined space. This typically involves the use of a harness and lifeline system that allows an incapacitated worker to be hauled out of the space from outside. The rescue plan must specify the type of harness and lifeline to be used, and the workers must be trained in its use before entry begins.
Entry rescue: If non-entry rescue is not possible, the rescue plan must specify the arrangements for entry rescue, including the rescue team, the rescue equipment, and the procedures to be followed. Entry rescue must only be carried out by trained and equipped rescuers.
Emergency services: The rescue plan must specify when the emergency services should be called and how they will be contacted. The location of the nearest hospital with an emergency department should be known to all persons involved in the confined space entry.
Communication: The rescue plan must specify how the entrant will communicate with the standby person outside the confined space, and what the standby person should do if communication is lost.

All persons involved in confined space entry, including entrants, standby persons, and supervisors, must receive appropriate training. The training must cover the hazards of confined spaces, the permit-to-work system, atmospheric testing, the use of personal protective equipment, and emergency procedures. The training must be provided by a competent person and must be refreshed at regular intervals.
